We understand that different patients have different needs, depending on where they are in their health journey. For this reason, we have developed a hybrid model. There are (3) distinct Care Delivery Pathways: Concierge Membership Primary Care, Urgent Care, and Specialty Care+. Specialty Care+ includes services like Cosmetic Neurotoxin (Botox, etc.), Male and Female Hormone Replacement Therapy, and Weight Loss Management, which are generally not covered by insurance. We also provide monthly medication and supplement packaging. These services are available to any patient for a fee.. A brief overview of each of these is given below. We will manage the vast majority of your medical issues and concerns and when a specialist needs to be involved we will remain active participants in your care. We have a very limited panel of patients for each physician, ensuring access always in a timely manner. An annual membership fee is charged and billed quarterly.
